1. Complete the form for each player on your team. Print legibly and make sure all information is filled out completely, including your team name. INCOMPLETE FORMS WILL NOT BE PROCESSED.
2. Include your team fee by filling out the credit card information or including a check or money order made payable to Streetball Louisville. Only accepting one check or money order per team. NO CASH ACCEPTED.
   REGISTRATIONS POSTMARKED BY MAY 26, 2006— THAT’S A “SLAM DUNK” FOR EARLY REGISTRATION — PAY ONLY $100.
   REGISTRATIONS POSTMARKED AFTER MAY 26, 2006 — THAT’S A “TECHNICAL FOUL” FOR LATE REGISTRATION — PAY $120.
   All registrations must be received (not postmarked) by June 9, 2006. Registration forms received after June 9 will not be processed. The final deadline is June 9 and this deadline will not be extended for any reason.
3. Include the signature of each player. ID will be verified at the tournament. False information will result in player and/or team disqualification without fee refund.
4. Include copy of photo ID (copies of birth certificates are permitted for children without photo ID) including date of birth and height of player. Parental or guardian signatures are required for children under 18 years of age verifying they have read and accepted the “Under Age 18 General Liability Release and Waiver of Claim” in this form.
5. To request “Open Division,” call the Streetball office at 502-636-1888.
